CAPABLE is a FREE program developed to help older adults live safely in their homes.

Services Include:

  • 8-10 total visits between a Occupational Therapist & Registered Nurse
  • Minor home repair, modifications and assistive devices

Eligibility Requirements

  • 60 years of age or older
  • Lives in Jefferson, Oldham or Bullitt County
  • Below 60% of Area Median Income
  • Must be able to set goals, and participate in an action planning process
  • Must have difficulty in performing activities of Daily Living (ADLs) such as bathing, dressing, grooming, or walking across a small rom
  • Requires minor home modification to ensure safety
  • Participant must be the homeowner or have permission from the homeowner to participate in the program and make modifications
