In-Home Care Services


This program assists persons 60 years of age or older who are at risk of becoming institutionalized or who are already in a facility but could live at home with assistance.  The goal of this program is to maintain elderly residents in their homes by providing needed assistance and case management. Services Include:

·         Assessment & Case Management

·         Homemaking -Housekeeping tasks such as light cooking, cleaning & laundry

·         Personal Care -Assistance with bathing, toileting and other activities of daily living

·         Escort -Accompaniment to physician, dentist or other healthcare service

·         Chore -Help with heavy household tasks

·         Respite -Relief service to regular caregiver

·         Home Delivered Meals


·         60 years or over

·  Functional disability (impairment in 2 physical activated of daily living or 3 instrumental activates of daily living)

·         At risk of institution care or in a nursing home or personal care facility

Fees are sliding scale based on client’s income
